Healthcare Passport

The new Healthcare Passport is a simple booklet that families and carers can fill in to share important details about a child’s health, likes, dislikes and the best ways to support them.  It makes sure children get care that really fits their needs.  

Families and carers can feel reassured that staff understand their child.

• It helps different services work together more smoothly.
• Information can be easily transferred over to the Purple Folder to support with transferring from CYP Services to Adult Services
• Families should take it along when their child sees new teams or moves between services.

Annual Health Checks for young people (14+) with disabilities

Young people over the age of 14 and on the learning disability register, are entitled to a free annual health check. This is a chance for a doctor to check overall health, and spot anything that may need early help, before it becomes a bigger problem. Annual health checks are not something that should be worried about, as they are a chance for a doctor to make sure young people are healthy.   A lot of young people with SEND find medical appointments a bit nerve-wracking. Annual health checks can help them to get used to attending appointments, and communicating their health needs.
